Improved Survival Rates with Fixed Bearing Total Ankle Arthroplasty

Introduction/Purpose: With increasing total ankle arthroplasty (TAA) being performed and a plethora of new survivorship data available, an updated literature review is needed to better understand the impact of different types of implants on the outcomes of TAA. The purpose of this study is to compare the outcomes of fixed vs mobile bearing TAAs in the literature. Methods: A comprehensive search of MEDLINE for all articles published between 2004 and 2023 was conducted with a minimum two-year mean follow-up. Two reviewers evaluated each study to determine whether it was eligible for inclusion and abstracted the data of interest. Meta-analytic pooling of group results across studies was performed, examining implant survival and component failure. Seventy-three implant groups met inclusion criteria (only 3rd and 4th generation implants), composed of STAR (20), Salto (mobile bearing) (10), Salto Talaris (11), Hintegra (8), Zimmer (6), INBONE II (6), INBONE I (4), Infinity (4), Cadence (4). 41 groups (56.2%) were implanted with mobile bearing devices, and 32 (43.8%) with fixed bearing implants. In total, 6498 subjects were included, with a mean age of 61.77 years and a mean BMI of 28.5 kg/m2. At a mean follow-up of 62.2 months, the overall reoperation rate was 21.13% and the metal component revision rate was 10.16%. Results: The mean follow-up of mobile bearing TAA studies (N =41) was 77.8 (range 24-188.4) months, and that of fixed bearing studies (N =32) was 45.3 (range 24-85.2) months. Multivariate analysis of bearing type, controlling for follow-up duration, found fixed bearing implants had statistically significantly higher metal component survival rates (fixed=96.1%, mobile=87.9%, p=.001); however, there were no statistically significant differences in rates of reoperation (fixed=17.0%, mobile=24.8%, p=.768). When examining specifically tibial and talar component failure rates, there was no statistically significant difference between bearing type (fixed tibial=1.46%, mobile tibial=2.95%, p=.472) (fixed talar=2.19%, mobile talar=2.79%, p=.966). Overall implant survival rates at one, two, five, and ten years were 96.4%, 96.0%, 92.7%, and 79.4%, respectively, and did not differ by bearing type (Table 1). Conclusion: Implant survival was statistically significantly better for 3rd and 4th generation fixed bearing TAAs as compared to mobile bearing TAAs, controlling for length of follow-up. There was no statistically significant difference in total reoperation rate. This suggests that fixed bearing implants may offer improved implant survival compared to mobile bearing implants, though further research is needed to confirm these findings.

Is There Still a Learning Curve for Primary TAA After Completing a High Volume Ankle Replacement Fellowship? A Multicentered Study

Introduction/Purpose: The learning curve in Total ankle arthroplasty(TAA) has long been reported. Surgeon experience of five to twenty-five ankle replacements have been shown to decrease various perioperative and long term complications in this complex surgery. Fellowship programs with high-volume ankle arthroplasty experiences have been suggested to mitigate this learning curve and provide benefit to early career surgeons with TAA outcomes. The purpose of this study was to evaluate the learning curve for TAA in two high-volume surgeons practices through the first three years of practice. Both surgeons completed fellowships and participated in more than 100 ankle replacements each. We hypothesized that experience during fellowship will help minimize complications and negate any learning curve previously established in the literature. Methods: Prospectively collected data was retrospectively reviewed on all patients who underwent primary TAA surgery during the first three years of clinical practice after fellowship in this IRB approved, multi-centered study. Patient demographics and clinical data, radiographic outcomes, and patient reported outcomes were collected on all patients. Patients undergoing revision TAA (as a first surgery), patients without a minimum of 2 years follow-up, and patients undergoing arthrodesis conversion to TAA were all excluded. The data was analyzed using Chi Squared test and the Moving Average Method where appropriate. All statistical analysis was performed by someone with an expertise in biostatistics. Results: Eighty-seven patients (67.8% male, avg. age 64) met inclusion criteria for the study. Average tourniquet time (111.9 min) and fluoroscopy time(106.3 secs) decreased as surgeon experience increased, with the this trend stabilizing after patient 20. Complication rate (wound complication, infection, return to OR, intraoperative fracture, etc.) was similar throughout the study period. No differences were seen in component positioning (postop AP TT angle, Postop Lateral TT angle), PROMIS Pain interference, PROMIS Depression, or PROMIS Physical function with regards to time during the study. Statistically significant differences (improvements) were seen in in the following postoperative measurements when compared to preoperative values: AP TT angle, Lateral TT angle, and PROMIS Pain Interference/Depression/Physical function. Conclusion: Learning curve as it relates to intraoperative fluoroscopy time and intraoperative tourniquet time remained even after completing a high volume TAA fellowship but clinical and PROs did not show this same trend. Radiographic measurements, complications (both intraop and postop), and patient reported outcomes did not show a time sensitive change after fellowship. Though some variables did show the maintenance of a learning curve, this multi-centered study suggests that completion of a high volume TAA fellowship can mitigate some variables in the learning curve. Impact of Surgeon Type on Total Ankle Arthroplasty Readmission, Complication, and Infection Rates

Introduction/Purpose: Total ankle arthroplasty (TAA) remains an attractive surgical option to address end-stage ankle pathology in appropriately selected patients. When successful, TAA can yield high patient satisfaction and marked improvements in patient- reported outcomes. However, surgical complications can lead to significant clinical impairment. TAA can be performed by both orthopedic surgeons and non-orthopedic surgeons – however, no prior study has compared TAA outcomes based on surgeon type. Methods: Utilizing Medicare claims from 2016 to 2019, we retrospectively identified patients ≥65 years of age who underwent TAA based on CPT coding. Patients on Medicare HMO, under age 65, and dual-eligible patients were excluded. Within this dataset, provider type was identified, allowing for comparisons between orthopedic and nonorthopedic surgeons. We compared groups based on readmission, all-complication, and infection rates within 1-year of TAA using logistic regressions controlling for age, sex, race, and comorbidity. To help mitigate bias, we also performed a propensity matched model with the same variables. Data were reported as percentage point (PPT) differences (95% CI) between groups, with a positive number indicating higher rate of complications among patients treated by nonorthopedic surgeons. Results: During our timeframe, 8,244 Medicare patients underwent TAA – 6,928 performed by orthopedic surgeons and 1,316 performed by non-orthopedic surgeons. There were no differences between groups based on age, sex, or race. Readmission rates were similar between groups (p=0.058). TAA performed by non-orthopedic surgeons had higher all-complication rates (43.4% vs. 36.7%, p< 0.001) and infection rates (8.9% vs. 4.7%, p< 0.001) compared to nonorthopedic surgeons. These differences in all- complication risk in non-orthopedic surgeons were further seen in logistic regression analyses (5.8 ppt (95% CI: 2.3, 9.4), p< 0.001) and propensity matched analyses (5.6 ppt (95% CI: 2.1, 9.2), p< 0.001). Group-differences were also revealed in infection risk through logistic regression analyses (3.9 ppt (95% CI: 2.0, 5.9), p< 0.001) and propensity matched analyses (3.8 ppt (95% CI: 1.8, 5.8), p< 0.001). Conclusion: TAA performed by non-orthopedic surgeons had greater all-complication and infection rates as compared to TAA done by orthopedic surgeons. Effects of Travel Distance on Complications and Outcomes in Total Ankle Arthroplasty.

Owing to the last decade's increase in the number of total ankle arthroplasty (TAA) procedures performed annually, there is a concern that the disproportionate distribution of orthopaedic surgeons who regularly perform TAA may impact complications and/or patient satisfaction. This study examines patient-reported outcomes and complications in TAA patients who had to travel for surgery compared to those treated locally. This is a single-center retrospective review of 160 patients undergoing primary TAA between January 2016 and December 2018, with mean age 65 (range: 59-71) years, mean body mass index (BMI) 28.7 kg/m2, 69 (43.1%) females, and mean 1.5 (SD = 0.51) years follow-up. Patients were grouped by distance traveled (<50 miles [n = 89] versus >50 miles traveled [n = 71]). There were no significant differences in rate or type of postoperative complications between the <50 mile group (16.9%) and the >50 mile group (22.5%) (P = .277). Similarly, there were no significant difference in postoperative PROMIS scores between the groups (P = .858). Given uneven distribution of high-volume surgeons performing TAA, this is important for patients who are deciding where to have their TAA surgery and for surgeons on how to counsel patients regarding risks when traveling longer distances for TAA care.Levels of Evidence: Level III: Retrospective Cohort Study.

BMI influence on total ankle arthroplasty outcomes: A systematic review

Total ankle arthroplasty has become an increasingly popular surgical treatment option for ankle arthritis. Patient selection for this procedure is a debated topic. Traditionally, total ankle arthroplasty is chosen for patients who perform only low-demand activities, those who are elderly, and those who are not overweight. More recently, foot and ankle surgeons have been expanding this selection criteria. The goal of the present study is to perform a systematic review on the impact of body mass index on total ankle survivorship and complications. 18 articles were included in the present systematic review. Five studies were analyzed for complications and indicated a potential increase in the odds of complications in patients with higher body mass index values, but this did not reach statistical significance (p > 0.05). There was not sufficient information to estimate a pooled effect of body mass index on total ankle arthroplasty survivorship. Due to the limited number of studies that met the review's inclusion and exclusion criteria, a statistical analysis was only performed on complication rates. In addition to a slight increase in complication risk for individuals with higher body mass index values, the results of this review also highlight the need for further studies that investigate the impact of body mass index on total ankle arthroplasty outcomes.

Outcomes after Total Ankle Arthroplasty with A Minimum Average Follow-Up of 10 Years: A Systematic Review and Meta-Analysis

Category: Ankle Arthritis Introduction/Purpose: End-stage ankle osteoarthritis (OA) is a debilitating condition that adversely affects quality of life and causes considerable functional impairment. Total ankle arthroplasty (TAA) is growing in popularity as a motion-preserving option for ankle OA. A previously published meta-analysis evaluated the mid-term outcomes of TAA with a minimum average follow-up of 5 years, demonstrating excellent survivorship. To our knowledge, there is no such manuscript that evaluates the outcomes of TAA at the long-term, with a minimum follow-up of ten years or greater. Due to a lack of long-term follow-up of TAA implants in the current literature, the aim of this review was to aggregate findings from the most up-to-date analyses of the long-term outcomes of TAA via meta-analysis. Methods: We conducted a systematic review and meta-analysis according to the PRISMA guidelines. Electronic databases were searched from the date of inception to September 12, 2022. These databases included Medline, Embase, and Scopus. We included all studies that 1) involved patients that underwent uncemented TAA, and 2) all patients who had an average follow-up period of at least ten years. We excluded abstracts, as well as manuscripts written in languages other than English. We collected American Orthopaedic Foot and Ankle Score (AOFAS) and Visual Analog Scale (VAS) scores from the included studies for pooled meta- analysis. Due to the varying definition of survivability between studies, this metric was not assessed in our final evaluation. Results: Our data included a total of 2,562 patients and 2,895 ankles with an average follow-up of 10 years. Of the 25 studies included in the systematic review, 7 provided pre- and post-operative AOFAS means and 5 provided pre- and post-operative VAS means with associated measures of variability and were included in our meta-analysis. The weighted mean difference between pre- and post-operative AOFAS scores was -41.98 (95% CI: -48.28 to -35.69) (Fig. 1A), indicating that patients improved by almost 42 points between pre-and post-surgery. The weighted mean difference between pre-and postoperative VAS scores was 4.52 (95% CI: 2.26-6.43) (Fig. 1B), indicating patients improved by about 4.5 points on average between pre-and post-surgery. Survivability was not assessed due to the heterogeneity in survivability definition between studies. Conclusion: Outcomes following TAA are favorable and indicate patient-reported outcomes improve over long-term follow-up. Future, prospective, randomized research should be conducted to better assess TAA, and authors should work towards standardization of survivorship and complication reporting methodologies to allow for pooled metanalysis of these important outcome metrics.

2023 Roger A. Mann Award Winner: Younger Patients Undergoing Total Ankle Arthroplasty Experience Higher Complication Rates and Worse Functional Outcomes Scores

Category: Ankle Arthritis; Ankle Introduction/Purpose: Although many patients with postraumatic ankle arthritis are of a younger age, studies evaluating the impact of age on outcomes of primary total ankle arthroplasty (TAA) reveal heterogenous results. The purpose of this study is to determine the effect of age on complication rates and patient reported outcomes after TAA. Methods: This was a retrospective study of 1,185 primary TAA divided into 3 age cohorts (Age &lt; 55, n=219; Age55-70, n=688; Age&gt;70, n=278). Patient demographics, intraoperative variables, postoperative complications, and patient reported outcome measures were compared among groups using univariable analyses. Multivariable Cox regression was performed to assess risk for implant failure by age group. Mean follow-up was 5.7 years. Results: Compared to Age55-70 and Age&gt;70 groups, Age &lt; 55 patients had the lowest mean ASA score (2.2 vs 2.4 vs 2.5; p&lt; 0.001) and highest rates of all complications (33.8% vs 23.1% vs 19.8%; p=0.001), implant failure (9.1% vs 6.0% vs 3.2%; p=0.022), and component removal (5.0% vs 2.8% vs 1.1%; p=0.030). Univariable and multivariable cox regression analysis revealed decreased risk for implant failure for Age&gt;70 compared to Age &lt; 55 patients (HR: 0.43 [95%CI: 0.19-0.94], p=0.034; and HR: 0.42 [95%CI: 0.18-0.95], p=0.038, respectively). Across the AOFAS Hindfoot and all FAOS subscales, Age &lt; 55 group reported the lowest (worst) mean pre and postoperative scores (p &lt; 0.001). The age &lt; 55 group had the highest mean VAS score at final follow-up (22 vs 14 vs 14; p&lt; 0.001). Conclusion: Studies involving large sample sizes with mid-to long-term follow up are critical to reveal age-related impacts on outcomes after TAA. Younger patients had higher rates of complications and implant failure and fared worse on patient reported outcome measures in the largest single-institution series to date evaluating the effect of age on outcomes in TAA.

A Systematic Review of Outcomes of Total Ankle Arthroplasty with INBONE II

Category: Ankle Arthritis; Ankle Introduction/Purpose: The INBONE II is a fixed-bearing implant with a stemmed tibial component commonly used for total ankle arthroplasty (TAA). Despite its frequent use and previously reported outcomes within individual clinical studies, no study has systematically reviewed outcomes of TAA with this implant. The purpose of this study was to perform a systematic review of the literature and report on outcomes of TAA with the INBONE II. Methods: A systematic review of the literature was performed according to the Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA) guidelines. Databases searched included PubMed, Google Scholar, SCOPUS and CINAHL from database date of inception through February 28th, 2023. Clinical studies of INBONE II outcomes in peer reviewed journals were included. Outcomes included were survivorship, patient reported outcome measures (PROMs), and complications. Outcome results were compiled using weighted averages when possible. This review of 26 articles included 808 patients with 667 primary TAA and 141 revision TAA. Results: Survivorship, with failure defined as metal component revision, was 97.8% at an average follow-up of 49 months (range, 14-85.4 months) and 94.2% at an average follow-up of 25.7 months (range, 9.1-47.5) months for primary and revision TAA respectively. Complication rates for primary TAA included: 3.8% intra-operative fractures, 2.5% post-operative fractures, 1.8% deep infection, 8.0% wound complications, 1.6% rate of poly exchange, and 1.6% nerve injuries. Revision TAA complication rates included: 12.3% intra-operative fractures, 6.9% wound complications, 3.3% deep infections, and a 5.5% incidence of nerve injury. Average PROMs for primary TAA included: AOFAS American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot score improved 41 points, visual analog scale (VAS) scores decreased 5.9, and patient satisfaction of 92.6%. Revision TAA PROMs were not consistently reported. Conclusion: This study is the largest compilation of INBONE II outcomes and survivorship. Primary and revision TAA with the INBONE II results in satisfactory improvement in PROMs and low complication rates. The overall survivorship is 97.8% and 94.2% at an average follow-up of 49 months (range, 14-85.4 months) and 25.7 months (range, 9.1-47.5) for primary and revision total ankle arthroplasty respectively. These findings support continued use of this implant in primary and revision TAA.

Intermediate Term Clinical Outcomes in Total Ankle Arthroplasty: The CU Total Ankle Registry

Category: Ankle Arthritis; Ankle Introduction/Purpose: Total ankle arthroplasty (TAA) is a common procedure to treat end stage ankle osteoarthritis. Despite recent increase in popularity, there is limited literature assessing clinical and patient-reported outcomes (PROs) in TAA procedures using 5 current generation implants. These measures are an important tool for physicians to better assess patient performance in daily activities. The purpose of this study is to characterize both PROs and clinical outcomes in primary and revision TAA surgeries using current generation prostheses performed by non-design surgeons at a single institution. Methods: Our institution maintains a prospective outcomes registry for patients undergoing TAA at our institution. Patient demographics, relevant case information, PROs and complications are collected. Collected PROs include PROMIS Physical Function (PF), Pain Interference (PI), Depression, Global Physical Health (GPH), Global Mental Health (GMH), and Foot and Ankle SANE. Records are categorized as a primary TAA, or revision involving either a polyethylene only exchange or complete hardware removal and replacement. Differences between groups were determined non-parametrically using Mann-Whitney U tests. In all statistical tests, a p value of less than 0.05 was considered significant. Results: A total of 259 patients were included for analysis; 223 primary and 36 partial or full revision. Average length of stay was 1.6 ± 1 days. There were 18 reoperations (6.95%), with no implant failures within 1 year. Five patients underwent implant revision at an average of 2.5 years. Of these, one was related to delayed infection at year 3, two due to hardware failure in year 2, and 2 polyethylene exchanges in years 2 and 5. All physical health PRO measures showed significant improvements post-operatively (p &lt; 0.0001). Patients reported significant reductions in pain following TAA at 3-months (Figure 1a), and significant improvement of physical function after 6-months (Figure 1b). There were no significant changes in Depression or GMH (Figure 1d). Conclusion: Total ankle arthroplasty is a safe and effective procedure for end-stage ankle arthritis. Patients experience improvement in all physical health PRO metrics at all post-operative time points after 3 months. Complication and early failure rates are lower than that reported in the literature. Long term outcomes and implant longevity will be tracked using the CU Total Ankle Registry. Total ankle arthroplasty improves quality of life in patients suffering from ankle osteoarthritis. Further follow-up and patient enrollment in the CU Total Ankle Registry will help us to identify risk factors for complications and define implant lifespan.

Increased Hospital Length of Stay for Obesity Class II and III Patients After Primary Total Ankle Arthroplasty: A NSQIP Analysis

Category: Ankle Arthritis Introduction/Purpose: Obesity is a well-established risk factor for complications after total hip and knee arthroplasty. The incidence of total ankle arthroplasty (TAA) continues to grow, but few studies have evaluated TAA outcomes in patients with obesity. The present study sought to measure the impact of obesity on TAA outcomes using a national database. Methods: The 2012-2020 National Surgical Quality Improvement Program (NSQIP) database was queried for primary TAA (CPT:27002). 1,610 patients were identified and stratified by preoperative body mass index (BMI): non-obese (18 5-29.9kg/m2, n=760), obesity class I (30.0-34.9kg/m2, n=455), and class II and III (&gt;35.0kg/m2, n=395). Preoperative and perioperative patient data, operative time, hospital length of stay (LOS), and 30-day complications were compared using univariable statistics. Multivariable analyses controlling for patient characteristics were employed to measure the effect of obesity on operative time and LOS. Results: Compared to the non-obese group, obesity class II and III patients were younger (61.9 versus 65.6 years;p &lt; 0.001), more female (57.2% versus 42.0%;p &lt; 0.001), higher ASA class (p &lt; 0.001) and had more comorbidities such as hypertension, diabetes, and dyspnea (p &lt; 0.001). Mean preoperative albumin was lower (p=0.013) and white blood count was higher (p &lt; 0.001) in the obesity class II and III group. Home discharge rate was lowest in class II and III patients (88.6%;p=0.017). Multivariable regression revealed longer hospital LOS for obesity class II and III (μ=0.21; p=0.005) relative to the non-obese group. Obesity group was not associated with operative time or 30-day readmissions and reoperations. Conclusion: Obesity class II and III patients undergoing TAA had more comorbidities and longer hospital LOS compared to lower BMI patients. Despite the well-established association between obesity and postoperative complications after hip and knee arthroplasty, the present NSQIP analysis of 1,610 TAA patients did not find a greater burden of acute complications among patients with obesity.

The Effect of Diabetes Mellitus on Total Ankle Arthroplasty Outcomes

Category: Ankle Arthritis; Ankle Introduction/Purpose: The utility of total ankle arthroplasty (TAA) for management of end-stage ankle arthritis continues to increase. Postoperative infection after TAA is a devastating complication leading to additional surgeries, hospitalizations, long term antibiotics, arthrodesis, and sometimes amputation. Diabetes has been shown to be associated with postoperative infections following other orthopedic procedures. While the majority of literature demonstrates an association between diabetes and post- operative complications following TAA, other studies have demonstrated no such association. The goal of this study was to determine if diabetes influences the outcomes following TAA. Methods: An insurance database was utilized to identify patients undergoing TAA for end-stage ankle arthritis with a concurrent diagnosis of diabetes based on CPT, ICD-9, and ICD-10 diagnosis and procedure codes from 2010 to 2021. The postoperative outcomes of all cause revision, periprosthetic joint infection, periprosthetic joint infection with revision, and aseptic revision were compared between diabetic and nondiabetic patients with a minimum 2-year follow up using Kaplan-Meier and Cox proportional hazards analyses. Patient demographics, comorbidities, and Charlson Comorbidity Index was queried and analyzed via univariate and multivariate analysis, along with 90-day postoperative outcomes. Results: The study population included 8,317 patients who underwent TAA, 345 of which had a concurrent diagnosis of diabetes, with a minimum of 2-year follow up from 2010 to 2021. After multivariate Cox proportional hazards analysis, the 5-year cumulative incidence of PJI was 7.3% in diabetic patients compared to 3.9% in nondiabetic patients, with a 95% increased risk of revision (HR 1.95, 95% CI: 1.15-3.30, P=0.01). Diabetic patients also demonstrated a 5-year cumulative incidence of septic revision rate of 1.4% compared to 0.4% in nondiabetic patients, with a 363% increased risk (HR 4.63, 95% CI: 1.22-17.52, P=0.02). After controlling for differences in demographics and comorbidities through multivariate analysis, diabetes was independently associated with an increased risk for stroke (OR 10.12, P=0.007). Conclusion: The incidence of post-operative complications, including increased risk of 5-year risk of periprosthetic joint infection and septic revision was higher among diabetic patients compared to nondiabetic patients.

Reliability of Cone Bean Weight-Bearing Measurements of the Postoperative Outcome of Total Ankle Arthroplasties and Inter-Method Reliability with Respect to the Weight Bearing x-rays

Category: Ankle Arthritis; Ankle Introduction/Purpose: Postoperative evaluation of the total ankle arthroplasty (TAA) alignment is a fundamental step in measuring the surgical outcome. The current reference standard, weight-bearing x-ray (WBXR), is subject to technical bias. Weight-bearing cone beam computer tomography (WBCT) allows visualization of the complex three-dimensional (3D) structure of the foot under the influence of its deforming forces. A WBCT-based measurement system to assess postoperative outcomes has yet to be validated. The purpose of this study was (1) to measure the surgical outcome of TAA using semiautomatic generated models, (2) to evaluate the agreement levels between two raters, and (3) to evaluate the inter-method reliability with respect to WBXR. Methods: Fifty-five consecutive patients were retrospectively viewed. Two raters independently created a 3D model using dedicated software. Each rater measured six different measurements: α angle tibiotalar angle, hind foot angle (HFA), β angle, γ angle, and Φ angle using a semiautomatic 3D model and one measurement, the tibiatalar ratio (TTR) using WBCT on sagittal view. Measurements were repeated two months apart in the same independent fashion. Results were compared to standard weight- bearing x-rays. Consequently, interobserver, intraobserver, and intermethod agreements were calculated. Results: All seven measurements showed good to excellent interobserver and interobserver reliability. (ICC 0.85-0.95). The intermethod agreement showed good agreement for the γ angle (0.79) moderate agreement levels for the α angle, TTS angle, β angle, and TTR (ICC 0.68, 0.69, 0.70, 0.69 respectively), a poor agreement for the HFA angle (ICC 0.25), and a negative agreement for the φ angle (ICC -0.2). Conclusion: Postoperative analysis of TAA using WBCT demonstrated good to excellent interobserver and intraobserver agreement and can be reliably used. When compared to WBXR, moderate to good agreement was measured for the sagittal and coronal alignment of the components. A poor agreement was measured for the coronal alignment of the hindfoot, and a negative agreement was measured for the axial rotation angle, indicating that the intermethod comparison is unreliable.

Early Osteolysis and Component Revision of CADENCE Total Ankle Arthroplasty at Midterm Follow Up

Category: Ankle Arthritis; Ankle Introduction/Purpose: Total ankle arthroplasty (TAA) for the treatment of end-stage ankle arthritis has continued to grow in popularity as a favorable alternative to ankle arthrodesis. Amongst the fourth-generation implants released over the past decade is the CADENCE Total Ankle System (CTAS) which became available for clinical use in 2016. Our previous publication revealed a concerning incidence of radiographic osteolysis of the CADENCE tibial component interface at short-term follow-up of 24 months. Though there were limited cases of symptomatic loosening or component revision in this report, there was concern this trend may worsen with longer follow-up. The purpose of this study was to evaluate the radiographic and clinical outcomes of TAA with the CTAS, with a minimum of 1-year follow-up. Methods: This single-center retrospective study evaluated 63 consecutive patients who underwent TAA with the CTAS between August 2016 and October 2021 by a single fellowship-trained foot and ankle surgeon and co-design surgeon. Patients were included in our study on the basis of at least one year of clinical and radiographic follow-up; there were no other exclusion criteria. Results: Fifty-four TAA cases utilizing the CTAS in 50 patients were included in the current study. Preoperatively, eleven ankles had neutral alignment, 18 ankles demonstrated varus alignment, and 25 ankles demonstrated valgus alignment. Twelve patients underwent staged procedures for management of severe coronal plane deformity. Radiographic parameters changed significantly preoperatively to postoperatively. At final follow-up, PPL was demonstrated in 39 (72.2%) ankles in this cohort. Symptomatic PPL was present in 13.0% of ankles in this cohort. Talar subsidence was observed in 7 ankles within our cohort. Nine ankles underwent subsequent operations related to TAA complications. Seven ankles underwent revision procedures, resulting in a final implant survivorship of 87.0%. Four ankles, 7.4% underwent non-revision reoperations. No additional complications were encountered within this cohort. Conclusion: In this study of 54 CADENCE TAA, we observed a high rate of component loosening and bone interface osteolysis over time. This ultimately led to poorer implant survivorship over time and a higher than acceptable revision rate. Though the lead author is a co-designer of the CADENCE implant, we believe it prudent to share our clinical findings and experience as we all gain further knowledge and understanding about implant design and TAA. Based upon our results, we have abandoned use of this prosthesis as the midterm results fail to achieve parity with other TAA systems available on the market.

Risks of Increased Operative Time and Longer Hospital Stays Based on Age in Total Ankle Arthroplasty

Category: Ankle Arthritis Introduction/Purpose: Total joint arthroplasty is increasingly being performed in younger individuals across the United States. While age-specific outcomes have been evaluated in total hip and knee replacements, the data are not as clear for total ankle arthroplasty (TAA). The purpose of this study was to analyze TAA outcomes and complications by age to improve shared decision-making when considering ankle replacements. Methods: A total of 1,619 primary and revision TAAs from 2012-2020 were collected and included from the National Surgical Quality Improvement Program. Patients were stratified by age into groups less than 55, between 55-70, and greater than 70 years of age. Demographics, medical comorbidities, ASA class, operative time, 30-day complications, and comorbid conditions were compared by univariable analysis. Multivariable analysis was used to analyze readmission rates, reoperation rates, operative time, and hospital length of stay. Results: By univariable analysis, patients greater than 70 have significantly more comorbidities when compared to patients younger than 55. Younger patients have a significantly longer hospital stay, with an average of 1.64 days for those younger than 55 compared to patients 55-70 (1.50 days) and patients older than 70 (1.38 days, p&lt; 0.001). Younger patients also have a significantly longer operative time and a higher risk of still being in the hospital after 30 days (p &lt; 0.001, p=0.034). By multivariable analysis, patients 55-70 have a statistically lower risk of readmission compared to the other two cohorts (p = 0.043). Conclusion: As the incidence of total ankle arthroplasty increases across all ages, it is important to understand specific risks for each age group. Patients younger than 55 experience higher operative times and longer lengths of stay. Patients between 55-70 have a significantly lower risk of reoperation but longer hospital stays. Patients older than 70 tend to have lower reoperation rates, lower readmission rates, and shorter operative times.

Outcomes of Total Ankle Arthroplasty in Postfracture Ankle Arthritis.

Ankle arthritis that develops after fracture accounts for a significant portion of ankle arthritis necessitating total ankle arthroplasty (TAA). It remains unknown whether TAA in postfracture patients produces equivalent outcomes to those without fracture history. The purpose of this study was to evaluate the medium-term outcomes of TAA in postfracture ankle arthritis compared to those without fracture history. This study reviewed 178 ankles from 171 consecutive patients who underwent TAA in our institution between 2007 and 2017 and completed a minimum 5-year follow-up. Four different TAA systems were utilized by 6 surgeons. Based on fracture history, patients were divided into 2 groups: the postfracture group (n = 63; median age 65.7 years; median follow-up 5.9 years) and the nonfracture group (n = 115; median age 64.4 years; median follow-up 6.2 years). Types and rates of complications including revision and reoperation were compared. Minimum 5-year Foot and Ankle Outcome Score (FAOS) and postoperative improvement were investigated. A subgroup analysis was performed to determine whether outcomes differ between intraarticular fracture patients (n = 43) and extraarticular fracture patients (n = 20). Both groups exhibited comparable postoperative improvement and final FAOS scores. The postfracture group had a significantly higher reoperation rate than the nonfracture group (20 of 63, 31.7%, vs 17 of 115, 14.8%; P = .011), with gutter impingement being the most common cause. There were 3 revisions in each group. In the subgroup analysis, we found no evidence of statistical difference between the intraarticular fracture group and the extraarticular fracture group in terms of FAOS scores, revision, and reoperation rates. In this single-center, retrospective comparative study, we found total ankle arthroplasty in patients with a history of fractures around the ankle joint had no evidence of statistical difference in patient-reported outcomes and implant survivorship but led to a higher rate of nonrevision reoperation following surgery. In the much smaller subset of patients with previous fracture, we did not find that those with a history of intraarticular fracture had inferior outcomes after TAA when compared to those with a history of extraarticular fracture. Outcomes after total ankle arthroplasty with an average follow-up of 10 years: A systematic review and meta-analysis

BackgroundThe literature demonstrating positive outcomes after total ankle arthroplasty (TAA) is mounting. However, the long-term outcomes of TAA (≥ 10 years) remain minimally reported. The purpose of this systematic review and meta-analysis was to evaluate outcome metrics over multiple TAA studies with greater than 10 years of average follow-up. MethodsTAA studies were searched in Medline, Embase, and Scopus from the date of inception to September 12, 2022. Inclusion criteria included 1) studies of patients that underwent uncemented TAA, and 2) studies with an average follow-up time of at least ten years. Manuscripts in non-English languages and isolated abstracts were excluded. We collected American Orthopaedic Foot and Ankle Score (AOFAS) and Visual Analog Scale (VAS) scores from the included studies for pooled meta-analysis. Due to the varying definition of survivability between studies, this metric was not assessed in our final evaluation. ResultsOur data included approximately 3651 patients (3782 ankles). Of the 25 studies with an average follow-up of 10 years included in the systematic review, 5 provided pre- and post-operative AOFAS means and 5 provided pre- and post-operative VAS means with associated measures of variability and were included in our meta-analysis. The weighted mean difference between pre-and post-operative AOFAS and VAS scores was −40.36 (95% CI −47.24 to −33.47) and 4.52 (95% CI: 2.26–6.43), respectively. The risk of bias was low to moderate for the included studies. ConclusionOutcomes following TAA are favorable and indicate patient-reported outcome improvement over long-term follow-up. However, a significant amount of heterogeneity exists between studies. Future, prospective, randomized research should focus on standardizing outcome measures, survivorship, and complication reporting methodologies to allow for pooled meta-analyses of these important outcome metrics.

Accuracy of a Patient-Specific Total Ankle Arthroplasty Instrumentation.

Total ankle arthroplasty (TAA) is a preferred surgical option for end-stage ankle osteoarthritis; however, it is a demanding procedure with a higher historical rate of revision compared with ankle fusion. Patient-specific instrumentation (PSI) has been introduced to optimize prosthesis alignment and theoretically overall improve TAA outcomes. The goal of this study is to report on the experience and surgical outcomes of one implant with specific evaluation of the accuracy and reproducibility of the system with respect to prosthesis alignment and prediction of implant size. A retrospective, multicentered study involving 4 foot and ankle fellowship-trained orthopaedic surgeon's patients undergoing TAA between January 1, 2015, and December 31, 2018, using the PROPHECY PSI system. 80 TAA procedures were performed. On average the postoperative tibial component alignment was 89.9 (range, 86.1-96.5) degrees in the coronal plane, with a mean sagittal alignment of 88.1 (range, 81.3-96.7) degrees. The mean deviation from neutral sagittal alignment improved from 4.9 ± 3.9 degrees preoperatively to 2.7 ± 1.7 degrees postoperatively, whereas the mean coronal alignment improved from 3.3 ± 2.5 degrees to 1.3 ± 1.1 degrees. The PSI software correctly determined the tibial implant size in 70 patients (89%). Prediction of talar implant sizing was less accurate than the tibial component, with 56 patients (71%) using the predicted sized implant. The overall implant survival at a mean follow-up of 45 months (range, 27-76) was 97.5%. We found that this PSI system accurately and reliably assisted in implant total ankle prosthesis positioning within a clinically acceptable margin and without significant outliers. Prediction of implant size was not as accurate as component orientation. Level III, retrospective study.

As the incidence of total ankle arthroplasty increases across all ages, it is important to understand specific perioperative risks for each age group. Younger patients tended to have longer surgeries and higher body mass indexes (BMIs). Patients aged 55 to 70 years had the lowest risks for perioperative complications. Patients greater than 70 years were at risk for longer hospital stays and discharge to nonhome locations. There were no differences in postoperative complications by age.
